New icons show available formats for each report

Icons beside each report now indicate available formats, making it easy to see which are available in an Excel-friendly format.

For reports available in both formats, users indicate the desired format on the Enter report parameters dialog box.

Appointments by referral source report now available in Excel-friendly format
