waiting room. Intake forms, medical history questionnaires, Ida Institute patient surveys or any
other custom forms sync back to Blueprint OMS, creating a better workflow and improving
your overall patient experience.
With online forms, you’ll have a collection of professionally crafted form templates for patient intake, medical history, marketing contact consent, and more.
We’ve also partnered with the Ida Institute to bring Ida Telecare tools to Blueprint OMS. These tools provide people with hearing loss easy-to-use tools and resources to help them prepare for appointments and successfully manage daily communication. Issue them to your patients directly from Blueprint OMS, and automatically archive their responses.

You can also send any form in Blueprint OMS to a tablet to collect a signature, no signature pad needed.
Once completed, online forms will automatically pass back into Blueprint OMS where they can be reviewed, processed, and archived by clinic staff.
Patient demographics and insurer info can create a new patient file, or update an existing patient record with a few clicks.

Enhance your patient care with new resources available directly in your Blueprint OMS. By popular request, new patient surveys are now available: the SSQ12* and the HHQ12.**
Like the IDA Institute surveys, these new surveys can be emailed before the appointment or issued to a tablet for completion in your clinic.
You can review the surveys in Setup > Templates > Forms: Speech, spatial, and qualities of hearing scale survey (SSQ12) and Hearing handicap questionnaire (HHQ12).
