New Tinnitus report template available!

We have received several requests for a dedicated tinnitus report template. In collaboration with a few of our audiologists, we have developed a professional tinnitus report template complete with placeholders for:

• Basic audiological test results
• Tinnitus Handicap Inventory (THI) questionnaire scores
• Tinnitus Functional Index (TFI) questionnaire scores
• Tinnitus assessment results
• Note space for medical history, assessment, and recommendations

If you would like access to this template, please click here to download it. 

In your Blueprint OMS, go to Setup > Templates > Forms. Click Upload. Select the downloaded template > Click Open. In the add document dialog box, select Type: ‘Audiological form,’ give the document a Title and optional Description > Click Save.

The report can now be accessed from the patient’s Audiology tab. It will auto-populate with clinic information, patient details, and audiological data.
