Emailing and faxing available for patient/physician correspondence

When generating correspondence, email and fax options have now been added.

Bug fixes

Create client privilege no longer allows viewing patient information

The Create client privilege now allows a user to only create a new patient file (but not view any other information within patient files).

To view the information within a patient file now requires the View client privilege.
